If you are unable to download and fill out the forms, please come in 15 minutes early to fill out the paperwork.

Cancellation Policy:

Life throws us unexpected surprises and we are expected to adapt to whatever comes at us. However, your scheduled appointment is yours and yours only. If you are unable to make your appointment for any reason, a 24-hour advance notice is requested. A changed, cancelled, or missed appointment without a 24-hour notice will be charged a fee.

Payment:

Payment is due at the time of service. For your convenience, cash, checks, and credit cards (visa, mastercard, discover & american express) are all accepted forms of payment.

Confidentiality Policy:

Patient confidentiality is of utmost importance, and because of that, confidentiality will be strictly maintained at all times as mandated by state, federal and HIPPA laws.
